申請插班學位須知

1.索取申請表方法

2.報名方法

將以下文件郵寄或親臨本校校務處遞交：

• ．已填妥的申請表
• ．香港身份證副本
• ．最近2年成績表副本
• ．相關之獎狀副本 (如有)
• (提交的副本不予發還)

3.校方以電話通知合適的申請人前來應試。

註：

1. 申請人及其家長切勿就其入學申請向學校教職員或法團校董會成員提供任何形式的利益。
2. 根據《個人資料（私隱）條例》，所提供資料將予保密及只用作本校處理入學申請之用。

2.Enrolment

Interested applicants are required to submit the following documents by post or by hand to the School Office.

• A completed application form
• A photocopy of HKID card
• Photocopies of report cards in the recent two years
• Photocopies of relevant certificates and awards (if any)
• (All submitted copies will not be returned to the applicants.)

3.Only shortlisted applicants will be notified for an admission test and/or

an interview by phone.

Remarks:

1. Applicants and their parents should not offer advantage to the school staff or Incorporated Management Committee members in connection with their applications.
2. According to the Personal Data (Privacy) Ordinance, the personal data you have provided in this form will be kept strictly confidential and it is only used for the purpose of processing the application of supplementary places.
